A Clear Contact Plan for Families and Caregivers
Knowing who to call can make everyday questions easier to resolve. Before care starts, agree on the family contact, office route and clinical responsibilities.
Choose a primary family contact
Several relatives may be involved, but conflicting instructions can make coordination harder. Decide who should receive routine updates and who can help communicate agreed changes. Respect the wishes and permissions of the person receiving care.
Keep the local office number available
Volusia and Flagler families use 386-767-5556. Seminole, Orange, Osceola and Brevard families use 407-878-0048. These existing numbers can help with new care, scheduling and caregiver questions. Ask how time-sensitive changes should be handled.
Identify clinical contacts
The person’s medical professionals remain responsible for diagnosis, treatment and clinical instructions. Keep the relevant contact details and current instructions available through the agreed process. A website message is not the right route for an emergency; call 911 for a medical emergency.
Agree on useful updates
Ask what information should be shared, how often and with whom. An update about a changed routine may be more useful than a vague reassurance. If the family wants an online care plan, ask the office about the available arrangement for authorized access rather than assuming this website is a patient portal.
Explore the help that fits together.
These pages explain different needs. Several kinds of assistance may be combined in one individualized care plan.

You do not need to have every answer before calling. Start with where care is needed and what is becoming difficult.
